Context

The CENPOS-PROJECT is based in Galicia, an autonomous community located in the northwest of Spain. The Regional Government of Galicia – known as Xunta de Galicia – is divided into various departments, one of which focuses on the social policy of the region. In 2013, the Social Policy and Youth Department collaborated with the Agency for the Technological Modernisation of Galicia (Agencia para la Modernización Tecnológica de Galicia – AMTEGA) to design a strategy for integrating new technologies into social services. The strategy led to the creation of the Trabe Plan to pursue the achievement of significant milestones, including the digitalisation of key processes of nursing homes and day centres for older people and the disabled. This was brought about by developing a new corporate management information ecosystem – CENPOS-PROJECT.

Objectives

The CENPOS-PROJECT encompasses a comprehensive stack of digital solutions that responds to the past, present and future transformations of the Department of Social Policy in the field of nursing homes and day care. The ecosystem incorporates several functionalities around five large digital solutions and more than 50 functional modules, which are specified in the following areas: (1)  a social and health station that offers key functionality to healthcare professionals, (2) an innovative system to support the operational processes of the facilities, (3) a solution for nursing home telecare, (4) a standards-based integration and clinical variable monitoring solution and (5) a data analytics solution.

Implementation

The project has had three phases of evolution: 2017–2018, 2019–2022 and 2022–2024, with a total investment of more than €8.7 million. For its development, the Government of Galicia has relied on the support of various IT companies, with more than 50 people participating in its development and implementation. The CENPOS-PROJECT offers functionality for the personalised care management of older people by creating an individualised care plan (ICP) for each person, comprehensively considering their medical, emotional, social and physical needs. This plan is carefully documented in the system, specifying the objectives and goals designed for the personal care of each individual. This includes organising activities, assigning therapies, prescribing medications, dietary recommendations and other personalised components intended to promote the resident’s overall well-being. In addition, the system enables a periodic review of these plans to ensure they are updated in the face of changes in the needs and progress of residents. To reinforce this process, the system is integrated with external platforms such as the Galician Public Health Service, allowing a bidirectional exchange of medical and pharmacological information, and improving the coordination between nursing home staff and the public health service.
